Welcome to the 15th MDG Pediatrics Clinic

Services

Our patient population ranges from newborn to 14 years.

  • Well Visits and Sports Physicals
    • Well-baby or well-child appointments are scheduled at ages 3-5 days, 2 weeks, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, and 24 months, and continue annually.
    • We can complete annual sports physicals during well visits, or backdate them to the last well visit, within 12 months. 
  • Acute Visits (Within 24 hours)
    • Because we see both well and sick children, our schedule is structured to see well children in the morning and sick children in the afternoon, to reduce the possible spread of infection.
    • You may book acute appointments by calling the appointment line or contacting the clinic directly through the MHS GENESIS Patient Portal.
 We don’t have urgent care capabilities. As a result, same-day appointments may not be available; seeking the nearest urgent care/ER may be advised.
 
  • Walk-in Procedures – Performed by nurses or technicians.
    • Mornings: 9 – 11 a.m.
    • Afternoons: 1 – 3 p.m.
  • Sore throat swab – Patient’s symptoms must meet clinical criteria before swabbing a patient for a rapid test and culture. Patients who don’t meet criteria are given guidance about viral throat infections.
  • Weight check – Patients enrolled in the 15th MDG and have been advised by their provider may obtain a weight check.
  • Suture/Staple Removals – Performed only if sutures/staples have been in place for the provider’s recommended time.
  • Wart Treatments – A provider must evaluate the wart prior to walk-in appointment.

 15th MDG Pediatrics does not yet have patient portal booking capabilities. You can send a message to request an appointment, and we’ll call you within 72 hours to schedule.

 Please advise front desk staff if you require any forms at the time of appointment.
 
The Survey of Wellness in Young Children is a comprehensive screening instrument from 2 months to 5 years. We verbally screen milestones of children 6 years and older at their appointment. Children 18 and 24 months additionally require the Modified Checklist for Autism in Toddlers.           
SWYC: 2 months – for children between 1 month, 0 days to 3 months, 31 days
SWYC: 4 months – for children between 4 months, 0 days to 5 months, 31 days
SWYC: 6 months – for children between 6 months, 0 days to 8 months, 31 days
SWYC: 9 months – for children between 9 months, 0 days to 11 months, 31 days
SWYC: 12 months – for children between 12 months, 0 days to 14 months, 31 days
SWYC: 15 months– for children between 15 months, 0 days to 17 months, 31 days
SWYC: 18 months– for children between 18 months, 0 days to 22 months, 31 days   
SWYC: 30 months– for children between 29 months, 0 days to 34 months, 31 days
SWYC: 36 months– for children between 35 months, 0 days to 46 months, 31 days
SWYC: 48 months– for children between 47 months, 0 days to 58 months, 31 days
SWYC: 60 months– for children between 59 months, 0 days to 65 months, 31 days
